Terms Of Service

• This plan is non-refundable. No refunds are given if the patient chooses not to use their plan within the year after it is purchased. The yearly cost is subject to change at renewal.
• All payments are due at the time of service to receive a 15% discount. If you chose to extend your payment for treatment by paying through CareCredit, the restorative treatment discount is reduced to 5% due to merchant fees
• Payment for membership can not be made with CareCredit or 3rd party financing 
• It is the responsibility of the member to utilize their benefits and arranging their appointments within the 12 month period of membership.
• The 12-month membership starts from the first date of service.  Payment must be paid in full.

What are the Exclusions?

*Periodontal treatment and additional periodontal maintenance cleanings will be billed at the 15 percent discounted price minus the reduced fee for the prophy. This is a dental discount plan and is NOT dental insurance

• This can not be combined with other discount plans or dental insurance
• The discount applies only to treatment, not to over-the-counter products
• It is good only at Huntingdon Dental Care, LLC. Therefore, if you are referred to a specialist, they will not give you this discount
• Should there be any treatment needed following any type of injury where a lawsuit and therefore outside medical, auto, disability, or workman's comp type insurance are involved, this discounted plan can not be used.
• This offer can not be combined with any other offer
• Plan fees are valid only when paid at the time of enrollment.
